Acknowledged as a population health practice by the Ministry of Health, we are entrusted with the responsibility of overseeing and delivering care to individuals through the appropriate health care provider channels—be it a family practitioner, nurse practitioner, nurse, or clinical pharmacist. This care is dispensed at the right setting, be it within our office, the comfort of your home, a long-term care facility, hospital, or even virtually. Our collaborative team approach ensures holistic and effective care for each patient, their families, and the larger community.
