About the Primary Care PTSD Screen (PC-PTSD-5)
This short questionnaire is designed to help your healthcare provider understand if you might be experiencing symptoms of post-traumatic stress disorder (PTSD). PTSD can occur after a person experiences a traumatic event, such as combat, sexual assault, natural disasters, accidents, or other dangerous situations.
The questionnaire asks about your experiences and feelings over the past month. It’s important to answer the questions honestly, so your healthcare provider can provide the best support for your care.
The PC-PTSD-5 is not a diagnosis, but rather a tool to help identify symptoms that may need further evaluation. If you answer “yes” to any of the questions, it doesn’t mean you have PTSD, but it may indicate that you should discuss your symptoms further with your healthcare provider.
