About the Generalized Anxiety Disorder (GAD–7) Questionnaire

The GAD-7 questionnaire is a tool that primary care providers use to understand if you might be dealing with excessive worrying or anxiety. It has 7 questions that ask about your feelings and thoughts over the past two weeks.


Anxiety is when you feel really worried, nervous, or uneasy about things, even if they might not be that big of a deal. The questions in the GAD-7 ask about how often you’ve felt these worries, if you’ve felt restless or on edge, and if you’ve had physical symptoms like a racing heart or sweaty palms when you’re nervous.

You’ll be asked to rate how often you’ve experienced these feelings, like feeling on edge or having trouble relaxing. Your answers can help your care provider understand if you might have anxiety and how intense it might be.


Being honest when you answer the questions is important so that the doctor can get a clear picture of what you’re going through and offer you the right support or guidance. Just remember, there’s no shame in feeling this way, and we are here to help you feel better.
